Description 

The purpose of this amendment is to provide Questions and Answers received in response to the Official Request for Standard Form 330 posted on December 4, 2015 and the subsequent amendment posted on December 21, 2015. The Official Request for Standard Form 330 has been revised accordingly to update Part V, Submission Information, and to reflect the evaluation of Section D under Technical Evaluation Factors 4 and 5. In addition, the DES Statement of Work (Attachment 1) has been revised to clarify the requirement for Scribe-compatible data deliverables under Task 2.2. The LOFC Clause and COI Clauses/Provisions (Attachment 2) has been revised to provide the language for the LOFC Deviation clause that the EPA anticipates including in the DES RFP and resultant contracts. The Official Request for Standard Form 330, Attachment 1, and Attachment 2 have been replaced in their entirety. Please refer to the revised documents posted on January 20, 2016. Attachments 3, 4, and 5 remain unchanged from the December 4, 2015 posting.
