AMENDMENT 0003 The purpose of this amendment is to answer additional vendor questions (see attached SF30). THIS AMNEDMENT MAKES NO CHANGES TO THE SOLICITATION. ::ORIGINAL:: The Environmental Protection Agency (EPA) hereby issues a Request for Proposal (RFP) for the erosion control and repair at the Mesa II Mine Site located in the Cove Chapter of the Navajo Nation, Apache County, Az. This solicitation is a Woman-Owned Small Business Set-Aside. Please see attached Solicitation and corresponding documents for requirements. Proposal are due no later than 15:30 PM PST, 05/31/2019. Telephone calls will not be accepted. All proposals are to be sent electronically via FedConnect.net. Questions are due no later than 15:30 PM PST, 05/10/2019. Questions received after this due date may not be addressed prior to the proposal due date. All questions shall be sent electronically via FedConnect. SET-ASIDE INFORMATION: Woman-Owned Small Business Set-Aside 8. SIZE STANDARD: 750 Employees 9. REQUIRED PERIOD OF PERFORMANCE: 105 days from issuance of Notice to Proceed (60 days remediation/construction activities and 45 days for as-builts/closeout documentation) 10. ANTICIPATED MAGNITUDE: Between $1,000,000 and $5,000,000 Site Visit/Pre-Proposal Conference: Date and Time: May 7, 2019 at 8:00am local time Location: We will meet at the Cove Chapter House located Rd. 33, 8 miles West of HWY 13/63 JCT in Red Valley, AZ. GPS: 36.584544, -109.190811 4x4 Vehicles will be required to reach the site. EPA will not provide any contractor personnel with transportation to the site. To register for the site visit please email the Contracting Officer at roe.autumn@epa.gov and provide the following information: - Company Name - Information for Primary Point of Contact - Number of participants Any resultant contract will be awarded in accordance with the solicitation procedures outlined in FAR Part 15, Contracting by Negotiation. All responsible woman-owned small business sources categorized under the stated NAICS Code may submit a proposal which shall be considered by the Agency. Notices or any modifications to this procurement announcement, including any amendments, will be posted using the FedConnect web portal. All interested parties who believe they can provide the required services must register with FedConnect at www.fedconnect.net/FedConnect/. There is no charge to use FedConnect.
