This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 140A0720Q0005 and is issued as a Request for Quote. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-04D. The solicitation is set-aside for eligible Small Business. The associated NAICS code is 621511. The contractor shall provide ON-CALL PHLEBOTOMY SERVICES to the Bureau of Indian Affairs, Office of Justice Services.
